Please Bring the Following
- A complete list of current medications, including any vitamins, herbal supplements
or over-the-counter medication.
- Your insurance card
If you are a new patient, please arrive 20 minutes early to complete paperwork,
and bring a copy of immunization and medical records.
Please contact your pharmacy to refill current prescriptions. Allow two
working days for medication refills.
You should receive notification of laboratory results by mail within two
weeks. If your results require immediate notification, we will call you.
Billing and Insurance
All insurance co-payments and deductibles are due at the time of service. Our practice
accepts most major insurance plans. If we do not have a contractual arrangement
with your plan, you will be expected to pay in full at the time of service. Please contact our office to determine if we accept your insurance plan. For billing
questions, please contact our centralized business office at 1-866-666-1279 from
8 a.m. to 4:30 p.m.
